Age 0 to 10 visa medical examination $180 (cash or debit).
Age 11 to 74 visa medical examination and lab $200 (cash or debit) plus X-ray $60 (cash only).
Age 75 and up visa medical examination and lab $250 (cash or debit) plus X-ray $60 (cash only).
Report summary is available upon request $ 30.
Extra fees may apply if more tests are required for certain medical conditions identified during the examination.
Refugee claimants with confirmed valid documents are free of charge. Please provide one of the following: IMM 5985, IMM 1442 or IMM 5965 for validation.

WHAT YOU NEED TO BRING

A valid government issued photo ID such as a passport (preferred), Ontario driver's licence, Ontario card or IFH refugee document.
All documents from the government regarding your medical examination such as an IMM 1017 form if available. Your UCI or UMI number if available. Applicants for up front medical examinations will have their file created in our office.
All previous specialist consultation reports translated into English if available.
All current medications with names and dosage translated into English.
Your glasses for visual acuity test.
Please come with a person who can translate and assist in filling forms if you are not fluent in English.

You do not need to fast for this examination.
You do not need to bring any photographs. We will take your photo upon arrival.
Be prepared to leave a urine sample if you are 5 years or older.
The x-ray facility is a 5 minute walk away with free parking.
The entire examination will take approximately 2 hours.
