WHAT SHOULD I BRING?
Please bring the following items with you on the day of your IME:

The IMM 1017 form.
All prescription medications, including their names and dosages (if applicable).
Government-issued identification, such as a passport or Canadian driver’s licence, is required.
Glasses for the vision test (if applicable).
A translator if your booked panel physician does not speak your language and you require one.
Payment.
