1)Original recent passport + xerox copy of the first & last page.
2)All Medical reports including current and previous prescriptions.
3)Relevant papers sent by the High Commission / University / Company
4)Records of previous medical history, prescriptions &discharge note of any surgeries
5)Please carry a PRINTOUT of the Covid vaccination record (All doses)
6) You can also bring your childhood vaccination records if this is available and should you consent to share this information during the Examination ( IME ).
KINDLY NOTE: Such a consent is strictly voluntary and not a mandatory component of the IME and will have NO BEARING on visa issuance or immigration admissibility to Canada. So there is no cause for any worry or concern should you have lost or misplaced the childhood vaccination records.Acceptable vaccination documentation must come from a vaccination record, either a personal vaccination record or a copy of a medical chart with entries made by a physician or other appropriate medical professional. 7) Kindly use a MASK in the Clinic premises as per Covid Protocol.

Note: a) You DO NOT need to come in a fasting state
b) Females clients: Please do not take any appointments during or just before the menstrual cycle since this may interfere with the results of the urine test
c) The total procedure can take upto 4 hours
