Please bring the following items with you on the day of your IME:

The IMM1017 form
All prescription medications, including their names and dosages (if applicable)
Government-issued identification, such as a passport or Canadian driver's licence, is required.
Glasses for the vision test (if applicable)
A translator if your booked panel physician does not speak your language and you require one.
Payment.

You also do not need to make any special preparations for your IME, such as fasting. You must, however, drink a glass of water one hour before your urinalysis appointment.
