1. Original passport and HKID Card

2. ***Immunisation Record*** - very important!

3. Consulate's referral letter (If applicable)

4. COVID-19 vaccination records (If applicable)

5. Spectacles or Contact Lens (if applicable) 6. Medical Treatment Details (if applicable)

7. Medical Report (e.g. Surgery report, pathology report, etc.) (if applicable)

8. Old Chest X-Rays and/ or TB Medical and Treatment Report (if applicable)

NOTICE:

1. Please reserve 90 - 120 minutes for the completion of medical examination.

2. Health examinee under age 16 must be accompanied by parent or legal guardian.

3. For pregnant lady, a medical certificate confirming pregnancy is required. 4. Please attend on time.
